- 1 legalized sheet of S1 Transcript
- 1 copy of Identity Card
- 1 copy of the Birth Certificate
- 1 piece of seal (Rp. 10,000)
- 2 pieces of 2×3 photos and 2 pieces of 4×6 colored photos with red background
- 1 piece of seal (Rp. 10,000) for signing a statement letter on the Registration Form
- certificate equalization letter from DIKTI (for overseas graduates)
